The staff at Dr. Charap's office want to provide our patients with the best possible service. To find out how we're measuring up to expectations, we encourage patients to give us comments on their visits. This input is so important to us that we have created the ability for patients to provide feedback through a form on this site while remaining anonymous. However, you must be in our patient email database to use the form.

If you have recently been to our office, we invite your comments and suggestions. To provide them, please enter your email address below. If you are in our patient email database, you will be provided a Patient Feedback Form. Your email address WILL NOT automatically be included with the form. However, you may identify yourself on the form via name and email address if you so chose. All fields on the form are optional.
